Verification Forms
Through the process of filing for federal financial aid students agree to provide any information that is requested or required by the U.S. Department of Education to verify eligibility for federal financial aid. The federal government automatically flags 30% of financial aid¬–receiving students at any institution for verification. This process is crucial and vital to you receiving your aid.
If your FAFSA report is flagged by the U.S. Dept of Education you will be asked to fill out additional paperwork. Each of the forms will be available here for you to download, print, fill out, sign, and turn into our office.

FERPA Release and Financial Aid Authorization Form
All student academic and financial aid records are maintained by Ohio Mid-Western College under the strictest of confidentiality in compliance with the Family Educational Rights and Privacy Act (FERPA) of 1974 (34 CFR, Part 99). This law dictates that no student record may be released to any person other than the student without explicit written and signed permission. Below you will find a link to both the FERPA Release Form and a Financial Aid Authorization Form. Both of these forms must be completed by the student each year to maintain confidentiality of records. The yearly requirement is also to be certain that information is kept up to date and that the student has complete control over the financial aid and academic records.
